The online form is preferrred since your information will be e-mailed
directly to the Registrar. With the online form you can pay by check (you'll get
a summary sheet to include when you mail your check) or by credit card (through
PayPal). Use the Printable form if you want
to take copies to, say, a branch or board meeting.
